1. Pharmacy Creates Request

The pharmacy submits the information need for the request (see article for more information).

2. We Contact the Office

If we need records do not have enough information for the request we will contact you online for medical records. You can send physical records (e.g. chart notes, labs) or complete one of our questionnaires. See the medical records article for more information.the office for the needed information. 

3. Outcome Reported Online

Once we have an approval or denial the outcome will be reported to you on the portal.
