Creating Requests Online

1. Create Request Online
Complete the questionnaire or attach documents for the request.
2. Send Prescription To Pharmacy
The pharmacy will check to see if an authorization is needed. If it is a different type of problem (e.g. deductible, refill too soon) they will contact the patient and resolve the issue.
3. Outcome Reported Online
Once we have an outcome it will be reported on the online portal.

1. Send Prescription To Pharmacy
The pharmacy will check to see if the prescription needs a prior authorization. You can also send records ahead of time with the request. If you do we will not need to contact you later for records.
2. Send Records
If we need records we will contact you online for medical records. You can send physical records (e.g. chart notes, labs) or complete one of our questionnaires. See the medical records article for more information.
3. Outcome Reported Online
Once we have an approval or denial the outcome will be reported to you on the portal.
