Create Request Process


1. Select Prior Authorization

To find the prior authorization request:

  • Select Pharmacy network, then prior authorization.
  • If you request prior authorization regularly it will appear on the homepage.


2. Complete the Form

The minimum information we need is:

  • Patient name
  • Date of birth
  • Drug (If not on the list include in the comment section)
  • Prescriber (search by name or NPI)
  • Pharmacy (search by name or phone number)

*Note: If you are a full service account you can select any pharmacy in the country.


3. Send Prescription and Add Documentation

Don't forget to send the prescription to the pharmacy selected! At this point you will be able to answer the questions or provide other forms of documentation.
