  • Submit missing information
  • Cancel Request

Submit Missing Information

Attach the missing information by screenshot or add a comment. You can review the minimum information we need for each request type below:

Once submitted. Select the "Information Submitted" option.

Cancel Request

Mark

To cancel the request select "Cancel"

using the options

on the right.

Add a comment if you would like the pharmacy to relay a message to the patient.
