1. Select Prior Authorization
To find the prior authorization request:
- Select Pharmacy network, the prior authorization.
- If you request prior authorization regularly it will appear on the homepage.
2. Complete the Form
The minimum information we need is:
- Patient name
- Date of birth
- Drug (If not on the list include in the comment section)
- Prescriber (search by name or NPI)
- Pharmacy (search by name or phone number)
*Note: If you are a full service account you can select any pharmacy in the country.
3. Paste And Submit
Go to the portal and paste the information:
- Use CTR+V to paste (or right click and select paste)
- Hit Save and Submit
